General Forms
You can use the following forms to submit applications in reference to your studies.
A PDF reader like Adobe Acrobat Reader is required to use the documents. The forms can be filled out on the computer before printing. Required information regarding the processing of personal data can be found in the data protection declaration (available in German only).
- Application for semester assessment / credit of examinations [DE]
- Application for compensation in the context of legal maternity protection [DE]
- Application for compensation due to impairment [DE]
- Application for additional statements on Diploma Supplement [DE]
- Verification of examination result / prerequisite [DE]
You can send the completed and signed forms by:
- email to zpa@tu-chemnitz.de
- dropping them into one of the Central Examination Office’s letterboxes (Reichenhainer Str. 70 in the Central Examination Office's area or Straße der Nationen 62 in the Student Service's area)
- mail service to the Central Examination Office (address: Technische Universität Chemnitz, Zentrales Prüfungsamt, 09107 Chemnitz)
